Welcome to the online data management system for the Prevention of Type 2 Diabetes Program

This online system has been designed to collect and manage the minimum data set for patients undertaking Lifestyle Modification Programs (LMPs). LMPs are a key component of the Australian Government’s Prevention of Type 2 Diabetes Program which is being implemented through the Divisions of General Practice Network. The system has been designed to be used by LMP facilitators, divisions of general practice, SBOs and AGPN to validate payments of the LMP subsidy and meet the program reporting requirements.
