Prevention of Type 2 Diabetes Program, as administered by UltraFeedback. If you

Account and login questions
I've forgotten my password or never received one.
There is a link on the front page that you can use to reset your password.
Can I choose my own password?
No. The system will not allow you to set your password nor retrieve an existing
password. If you reset your password, it will be a randomly generated, strong
password.
I've also forgotten my username or never received one.
If you should have access to the data capture system, please contact AGPN or
UltraFeedback to have your account refreshed or created.
I tried to reset my password but received an error message.

Recording patient data and outcomes
How can I find a patient I've previously entered? The table of patient records on the Provider page gives you a list of patients for whom you have recorded a data set and who have not yet completed programs.
This list is ordered in reverse chronological order: the most recent attendances
are shown first. You can also use the Search boxes to search by any combination
of family name, first name and postcode. Finally, entering a Client ID into the
Client ID box will retrieve a record for that specific patient if they are in
your programs.
The search function retrieved the wrong patient's details.
If the Search button took you directly to patient data form, you probably used
the Client ID field. Using this field bypasses the other search fields.
Why can't I retrieve details I've previously entered? I need to change some
results.
Unfortunately, the system does not currently allow for results to be altered
once they've been entered. Each time you select an existing patient record,
you'll be prompted to enter new clinical results for them. This prevents
accidental changes to existing data. Important changes can be made by
UltraFeedback if required.
How can I change a patient's details? I click "Submit" but the changes aren't
recorded.
You can change the postcode and the concession card status of a patient. The
rest of the patient's details are not able to be changed. If you need to change
a patient's details - referral source, name, etc., you'll need to contact
UltraFeedback.
My patient has changed program. Can I change the LMP they are enrolled in?
For a patient to successfully enrol in a LMP, they will need to have a new
record created. You cannot change an existing patient record to attribute them
to a different program.
What is the "Workhealth Victoria" box for?
Employees of Workhealth Victoria are being funded to enrol in Lifestyle
Modification Programs. Selecting this flag marks a patient to be included in
specific reporting for this group.
What does "Funding Eligible" mean?
Most patients enrolled in LMPs will be eligible for subsidy from the central
funding for the Prevention of Type 2 Diabetes program. Some patients will not be
eligible - if they are not referred using one of the three medicare referral
items, they are outside of the specific age ranges, or they are enrolled in LMPs
as private patients. If "Funding Eligible" is not selected when the patient's'
record is created, no funding records will be created for these patients.
Why are there separate "Enrolment" and "Commencement" buttons?
Some program providers may wish to enrol patients and record the initial data
set from the GP Referral Form, but record a separate set of data at the
commencement of the LMP. This allows the LMP to accurately report any changes
from the beginning of the program to its end. If you create an initial data set
upon enrolment but do not specify "Commencement", a separate data set for
Commencement will be required before completion of other phases of the program
will be available.
